Consultant Pharmacist

We are seeking a Consultant Pharmacist to provide exceptional clinical services, support patient transitions of care, and collaborate with healthcare teams in a variety of care settings. Qualified candidates must either hold or be in the process of obtaining pharmacist licensure in Minnesota and Wisconsin, and any other states requested by the Director of Clinical Services. Candidates must also be open to a variable schedule, including weekdays and weekends, to accommodate facility needs.

Responsibilities:

  • Provide Medication Management Services, including transitions of care reviews, medication regimen reviews, interim medication reviews, medication reconciliation, drug regimen reviews, and disease state management reviews.
  • Conduct medication regimen reviews at the time of request, monthly, or per patient acuity level, and report findings in writing to the facility administrator, director of nursing, and physician.
  • Act as a liaison between facility and pharmacy staff, as well as between the facility, resident, and provider.
  • Engage in customer retention activities with facility staff.
  • Assist in the development of emergency drug supply kits in conjunction with the medical director, provider pharmacy, and nursing administration.
  • Attend quality assurance and other committee meetings as requested.
  • Prepare quarterly quality assurance reports.
  • Provide education to residents preparing for discharge, either directly or indirectly.
  • Offer medication counseling in person or via teleconference to residents in care facilities or at home.
  • Plan and present in-service education to pharmacy staff and nursing facility employees as needed.
  • Stay informed of and implement relevant pharmacy policies & procedures.
  • Maintain knowledge of CMS regulations, interpretive guidelines, and applicable state and federal pharmacy laws.
  • Pursue continuing education relevant to the practice setting.
  • Supervise field technicians as appropriate.
  • Perform additional duties as assigned.

Qualifications:

  • B.S. Pharm. or Pharm D. degree from an accredited School of Pharmacy.
  • Current pharmacy license in the state(s) of practice.
  • Experience in Long-Term Care Pharmacy or Consulting in Long-Term Care.
  • Valid driver’s license without restrictions.
  • Professional demeanor with strong organizational and clinical pharmacy skills.
  • Excellent oral and written communication abilities.
  • Proficiency in learning new technology applications.
  • Flexible 7-day availability to accommodate facility requests.
  • Willingness to travel within the territory, including occasional overnight stays.
  • Ability to work independently and as part of an interdisciplinary team to deliver high-quality resident care.
